The designated forms should be printed out and completed prior to visit at your local University Imaging Center.  To ensure adequate preparation for your exam, read all preparatory instructions related to your procedure.  Please bring a valid drivers license and insurance card.  Appointments can be made up to 24 hours before visit.  Please contact your local center for a telephone confirmation and screening.  We believe in serving you effectively, efficiently, and as timely as possible.
